Do you want to file a complaint, compliment or question?
If you want us to respond, please fill in the form as complete as possible. If it is a complaint about the treatment of a patient please fill in the personal details of the patient. Try to describe your complaint as concrete as possible. Which date or period did it happen, in which facility did it happen and which member of staff was involved? We need to know this information, because otherwise we can't conduce your complaint.

In order to handle your response carefully, we ask you to enter some personal details. In all cases, the employees involved ensure that your complaint or suggestion is noted and treated.
If you have filled in a form, you will always receive a confirmation. If you appreciate this, you will also receive a personal message afterwards about the handling of your complaint or suggestion. In order to be able to respond effectively to suggestions for improvement, we keep a record of all notifications that reach us.
Discuss or file a complaint in writing?
You can express your dissatisfaction or complaint to the person concerned or his department head/manager. They are happy to discuss this with you and try to find a solution as soon as possible. In such a conversation you solve misunderstandings together. Is conversation with the person(s) not possible or is it not satisfactory? Then contact the manager(s) of the relevant department or the management of the organization that includes the person or department concerned.
